“ayokong maligo diba bawal yun?” as verbalized by the client Objective: Unkempt hair Oily skin

Knowledge deficiency related to information misinterpretation as evidenced by statements reflecting misconceptions.

After 2 hours of nursing intervention, the client will verbalize understanding about the importance of taking a bath with emphasis on perineal hygiene and perform necessary procedures correctly and explain reasons for the actions.

Assess ability to learn or perform skill Explain to the client about the importance of taking a bath and perineal washing. Explain to the client the risks of not taking a bath and not having perineal washing. Differentiate fact from her belief. Assist client to take a bath. Assist client to perform perineal washing.

Cognitive or physical impairments need to be identified so an appropriate teaching plan can be designed. This will allow the client to understand the problem thus, this will make her cooperate. Immediate feedback allows learner to make corrections rather than practicing the skill incorrectly.

Goal was partially met. The client verbalized understanding as evidenced by “sige maghuhugas ako akala ko kasi bawalpa” Perineal washing was done but she was not able to take a bath.
